IMPORTANT NOTICE

I, the parent or guardian of the registrant, a minor, agree that the registrant and I will follow the rules of 7eliteSports and its partner schools. I understand that participation in a physical activity carries a risk of injury and that, by enrolling my child in this program, I accept that risk on their behalf. I authorize 7eliteSports staff to arrange transportation to and from program activities where applicable, and I understand that photos or video of my child taken during program activities may be used on 7eliteSports’ website or promotional materials unless I notify the program office in writing.

MEDICAL RELEASE (MINOR)

I give permission for my child to take part in 7eliteSports programming. I understand the normal risks involved in youth sports and physical activity, and I release 7eliteSports, its coaches, and its partner schools from liability for injuries that are not the result of negligence. I authorize program staff to seek emergency medical care for my child if I cannot be reached, and I agree to be financially responsible for any related medical expenses.

WAIVER & RELEASE

Youth sports carry some risk of injury no matter how carefully a program is run. That’s true of soccer, flag football, basketball, track, volleyball, and tennis alike. Most injuries are minor — scrapes, sprains, bruises. Less often, a child may suffer something more serious, like a broken bone. Rarely, a serious accident can occur. Registering your child means you understand and accept this.

Coaches are expected to enforce basic safety rules at every session — proper warm-ups, age-appropriate drills, and stopping play if a field or court isn’t safe to use. Parents are asked to let us know before the season starts if their child has a condition that could affect their ability to participate safely.

By signing below, you confirm that you’ve read this notice, that your child is physically able to take part in the sport selected above, and that you voluntarily accept the risks described here on behalf of your child.
